Sports Physical Appointments: Appointments are required to get a Sports Physical exam at the Teen Health Center. Dates vary each month for when these exams are offered. Call or stop by the school-based clinic to get more info, to pick up required paperwork, and to schedule an appointment. Exams are free of charge.
Sports Physical exams take approximately 20 minutes. There are a limited number of before school and lunchtime appointments available, so schedule early. Teens should bring eye glasses to the appointment (if they wear them) as a basic eye screen is part of the exam. Parents are not required to be present for the exam.
Required Paperwork: The Juneau Teen Health Center requires that a Sports Physical Health History form (see link below) be completed and turned in to health center staff PRIOR to the appointment. Parents and athletes complete the form together and both sign the form at the bottom of page 2. Please complete the form carefully and thoroughly as it provides important health information needed for a comprehensive exam.

A copy of the 2-page Juneau Teen Health Center Sports Physical Health History form is pictured below and can be downloaded using the above link. The Juneau School District and ASAA provide a different Physical Exam form available at each high school's Activities Office that can be used if you choose to have a Sports Physical exam completed by YOUR family doctor.
NOTE: It is recommended that athletes get Sports Physicals annually. Planet High School has validity dates.
